I had one break about 45 years ago. Between my dad and me, we made one out of steel. Not too hard to make if you still have the broken one to make a template.

I use dippers for the majority of my loads. Either Lee or I make my own from empty cases. Fast, accurate, and never noticed any difference on target. Just my experience
This is one reason I use volume measures. The powder column will be the same height in the case, regardless of weight
I'm inclined to think it was fired in a rifle chamber. If they were .45 Colt I'd have no doubt. Some of the pistol caliber long guns have yuge chambers.
